- comments: observed no city of chicago certified food manager on the premises while tcs foods (eggs, pork) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing soap at the dish area handwashing sink. All handwashing sinks must be equipped with soap at all times. Instructed to provide. Priority foundation 7-38-030-(c). Citation issued.
25. Consumer advisory provided for raw/undercooked food
- comments: observed an incomplete consumer advisory for raw or undercooked foods; advisory is missing a disclosure statement and multiple raw/undercooked menu items are missing an asterisk. Instructed to identify raw/undercooked foods by asterisking them to a foot note that states: 'items can be served raw or undercooked or contain undercooked ingredients' and add a statement that says: 'consuming raw or undercooked animal products may increase your risk of food borne illness, especially if you have medical conditions'. Priority foundation 7-38-005.
38. Insects, rodents, & animals not present
- comments: observed approximately 60 rat droppings throughout the garage dry storage area; on the floor underneath the metal storage racks, by the fryers and equipment, and on top of a table. Management instructed to clean and remove all rodent droppings from the garage and sanitize affected areas. Also observed approximately 20 ants in the office, on the floor near the door frame. Instructed management to contact pest control for service to eliminate all pest activity from the premises. Priority foundation 7-38-020(a). Citation issued.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard being used as shelf liner in the garage dry storage area. Instructed to remove the cardboard so surface is smooth, non-absorbent, and easily cleanable.
51. Plumbing installed; proper backflow devices
- comments: observed a leak from the faucet at the front cashier/coffee area handwashing sink. Must repair.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle (waste can with lid) inside the unisex washroom or the garage washroom. Instructed to provide a covered waste receptacle for feminine hygiene products inside each washroom.
55. Physical facilities installed, maintained & clean
- comments: found heavy dirt accumulation on the floor of the garage area, the floor inside garage washroom, and the floors inside the walk-in cooler and freezer. Instructed management to clean and sanitize said areas and maintain.
60. Previous core violation corrected
- comments: previous core violations from 3/3/2021 report # 2492347 not corrected: violation #56 (6-304. 11) must provide working ventilation fan in the garage washroom and must maintain. Violation #58 (2-102. 13) observed one of the certified food manager without an allergen training certificate. Must show proof of training for all certified food managers. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
