- comments: 6-202. 15 observed rear door open without a screen. Instructed to provide screen and make door rodent proofed when opened.

- comments: 6-202. 13 observed open bait stick traps being set in the basement. Instructed to only allow pest control to set all pest control devices.

- comments: 3-305. 12 observed liquor bottles stored on shelves in the basement under sewer lines. Instructed to remove and do not store any food, beverage or single service items under sewer lines.

40. Personal cleanliness
- comments: 2-402. 11 observed a male food handler without a beard restraint. Instructed to provide.

- comments: 4-601. 11(c) observed accumulated food and grease debris under the flat top grill and in between cooking equipment and refrigeration units. Instructed to clean.

- comments: 5-103. 11(b) observed inadequate hot running water at the sinks throughout the entire facility; 75f at the 3 compartment sink and hand washing sink in the food prep area, 82f at the 3 compartment sink and hand washing sink at the bar area and 70f in the toilet room. Instructed to maintain all 3 compartment sinks at 110f at all times and a hand washing sinks at 100f at all times. Priority violation. Citation issued 7-38-030(c).

Observed about 75 rodent (rat) droppings scattered in various areas of the basement, and about 20 rodent (rat) droppings on the top shelf of the liquor shelves. Instructed to clean and sanitize affected areas and reccomended contacting pest control for a service. Priority foundation. Citation issued 7-38-020(a). Observed about 100 rodent (rat) droppings scattered in the basement during the re-inspection. Instructed to remove rodent droppings, clean and sanitize affected areas and instructed to contact pest control for a service. Priority violation. Citation issued 7-42-090.
