- comments: observed no paper towels available at the hand washing sinks in front and rear bar area. Instructed to provide paper towels at all hand washing sink at all times. Person in charge provided paper towels during inspection. Priority foundation violation 7-38-030(c) citation issued.

- comments: observed no soap at the hand washing sinks in front and rear bar area. Must provide hand soap at the hand washing sinks at all times, person in charge provided soap during inspection. Citation consolidated with the violation above. Priority foundation violation 7-38-030(c)

- comments: observed no hand washing signage in front bar area exposed hand washing sinks. Must provide and maintain.

- comments: 8-103. 12(b) observed facility preparing sushi rice on site without a sushi rice laboratory letter nor a ph meter on site to properly test the sushi rice. Instructed to provide. Manager discarded about 2 lbs of sushi rice during the inspection. Priority foundation. Citation issued 7-38-005.

- comments: must properly label food storage containers in rear storage area.

- comments: observed evidence of improper pest control inside premises. Observed approximately 60 small live flies on garbage cans and wall next to ice bin in front bar area, on walls in rear dish washing area and rear storage shelving. Must clean and sanitize areas. Additional pest control service is needed to eliminate flying insect problem. Priority foundation 7-38-020(a) citation issued.

- comments: observed wiping cloths stored on top of ice bins in front bar area and rear food preparation area on prep tables. Must properly store wiping cloths in a sanitazing solution when not in use.

- comments: observed clutter and unnecessary items in rear storage area. Must remove clutter and organize area to prevent rodent harborage.
