- comments: observed person in charge without his city sanitation certificate as required.

2. City of chicago food service sanitation certificate
- comments: observed no valid city of chicago certified manager on duty while tcs foods such as chicken and fish are being prepared,handled and served. A certified manager must be on site and valid certificate posted at all times tcs food prepared and served. Priority foundation #7-38-012 citation issued.

49. Non-food/food contact surfaces clean
- comments: must clean the interior of the coolers, counter top, storage shelving and prep tables to remove spills and/or debris.

51. Plumbing installed; proper backflow devices
- comments: observed a leaky pipe underneath the 3 comp sink in the front prep area. Must repair and mainatin.

55. Physical facilities installed, maintained & clean
- comments: floors need cleaning along the wall base and in corners through out to remove spill, debris and/or grease.

56. Adequate ventilation & lighting; designated areas used
- comments: observed the hood above the cooking equipment dirty. Must clean to remove the grease and dust.

58. Allergen training as required
- comments: instructed manager to comply with the allergen training requirements or citations will follow.

64. Public health orders
- comments: observed 2 employees (the cashier and cook/manager) not wearing any face covering as require. Must comply. Citation issue #7-28-060.
