1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed food service manager without valid certification.

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago food service manager certification on site for food service manager. Instructed to obtain and provide on site. Priority foundation violation 7-38-012 citation issued.

37. Food properly labeled; original container
- comments: observed brute garbage cans being used to store flour and fish coating for foods. Instructed to provide food grade containers only and containers must be labeled with the common name of whats inside the containers.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard lining the shelving units in the rear and some form of ply wood in the dish washing area. Instructed to remove all.

- comments: observed the inside of the deep fryer oil filtering units with a heavy build up of grease and food debris. Instructed to clean and maintain all.

- comments: observed a heavy accumulation of grease build up and food debris on all of the cooking equipment. Deep fryers with a very heavy grease build up. Instructed to clean and maintain all as a means of preventing possible contamination of foods being prepared.

- comments: observed leak at the faucet and sprayer hose of the three compartment sink in the rear. Also at the handwashing sink located in the front food service area leaking from the bottom drain pipe. Instructed to repair and maintain.

- comments: observed a heavy build up of dirt, dust, food splatter and debris on the walls and floor of the facility in the front food service area and in the rear storage/dish washing area. Very heavy build up of grease on floor near the grease tank (informed that it overflows) instructed to clean and maintain all areas.

- comments: observed large holes in ceiling tiles in rear near the mop sink, with mold and badly damaged, some coverings missing. Missing ceiling coverings in the dish washing area and badly stained coverings in the front food service area. Instructed to repair/replace all as needed.
