- comments: 6-301. 14 must provide hand washing signs at all hand washing sinks.

- comments: the high temperature dish machine is not in use at this time. Must clean and maintain the unit. Priority 7-38-025.

38. Insects, rodents, & animals not present
- comments: 6-501. 112 observed dead cockroaches on glue traps in the food prep area. Also observed dead cockroaches on the prep tables and on the floor of the prep area. Must clean and remove all dead cockroaches.

39. Contamination prevented during food preparation, storage & display
- comments: 3-307. 11 must install a splashguard in between the exposed hand washing sink in the rear food prep area and the steam table.

- comments: 4-903. 11 observed single use items stored improperly on the storage shelves in the rear food prep area. Must store single use food storage containers inverted to protect against contamination.

- comments: 4-602. 13 clean the interior of the dishmachine, deep fryers, prep tables, food storage shelves, interior and exterior of ovens, interior of prep coolers, encrusted grease on the wok station and light shields in the food prep area, interior and exterior of the high temperature dish machine.

- comments: 6-304. 11 observed the gas bread oven and gas 1 burner stove both not stored underneath the ventilation hood. Management instructed to store all cooking equipment beneath the ventilation hood.

- comments: 8-404. 13(b)(3) previous priority foundation violation from report #2522727 on 7/14/21 not corrected: observed evidence of insects and rodents on the premises in the food prep area. Observed approximately 25 live small flying insects and house flies in the food prep areas, dining room, and storage areas. Also observed 3 live cockroaches between the corner drywall and frp wall of the food prep area, 1 live cockroach on plastic wrap dispenser and one on the wall. Also observed approximately 100 mouse droppings on the floor and on equipment/pipes in the utility closet, inside the deep fryer cabinet door and on the lower ledges of the prep line cooler. All rodent droppings must be removed and surfaces cleaned. Eliminate pest activity. Additional pest control service is needed to eliminate the pest activity. Priority 7-42-090. Citation issued.
