- comments: 6-501. 112 observed dead cockroaches on the prep tables and on the floor of the prep area. Must clean and remove all dead cockroaches.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: 4-302. 14 observed no chemical test strips on the premises to measure the final rinse concentration at the dish machine or the 3 compartment sink. Must provide chemical test strips for chlorine to test the sanitizer concentration. Priority foundation 7-38-005.
49. Non-food/food contact surfaces clean
- comments: 4-602. 13 clean the deep fryers, prep tables, food storage shelves, interior and exterior of ovens, interior of prep coolers including around the door gaskets, encrusted grease on the wok station and light shields in the food prep area.

- comments: 6-501. 12 clean the grease and food debris on the floor throughout the food prep and storage areas. Clean the food debris around and inside of all floor drains. Clean the grease and food debris on the baseboards behind the cooking equipment. Clean the wall beneath the 3 compartment sink.

- comments: 6-501. 11 seal the openings in the corner wall around the mop sink and along walls in the food prep area so that the area does not harbor pest activity.

- comments: 8-404. 13(b)(3) previous priority foundation violation from report #2522727 on 7/14/21 not corrected: observed evidence of insects and rodents on the premises in the food prep area and utility closet. Observed 2 live small flying insects in the food prep area. Also observed approximately 15 live cockroaches on the walls, floor and on equipment in the food prep areas and dish washing area. Also observed approximately 10 mouse droppings on the floor of the utility closet and behind the hot water tank. All rodent droppings must be removed and surfaces cleaned and sanitized. Additional pest control service is needed to eliminate all of the pest activity. Priority 7-42-090.
