1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no one in charge who demonstrates food safety knowledge on site. Priority foundation per 2-101. 11

2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certifed food manager on site while tcs food including fish and chicken are preparred cooked and served. Priority foundation violation 7-38-012 citation issued

3. Management, food employee and conditional employee; knowledge, responsibilities and reporting
- comments: observed no employee health policies on site for food handlers. Priority foundation violation 7-38-010 citation issued

5. Procedures for responding to vomiting and diarrheal events
- comments: observed no written procedure and no adequate supplies on site to properly respond to vomit and/or diarrheal events. Instructed the person in charge to maintain a kit with all necessary supplies including a sanitizer rated as effective against norovirus on site at all times. Priority foundation violation 7-38-005. Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the only 2 handwashing sinks located within prep/cooking area and dishwashing area blocked and fully inaccessible for handwashing. One sink was blocked by a hose attached to sink creating a cross connection and a chair in front of sink. The other sink was filled with cleaning supplies and also blocked by a large garbage in front of sink. Handwashing sinks were made fully accessible during inspection. Priority foundation 7-38-030 (c) citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 25 rat droppings in basement closet containing hot water heater. Remove all rat droppings. Clean and sanitize all affected areas. Priority foundation violation 7-38-020(a)

- comments: observed no pest control log book on site. Must obtain a contract with licensed pest control service with complete log book. Priority foundation violation 7-38-020 (c)

49. Non-food/food contact surfaces clean
- comments: shelving in basement for food and food related dry storage items contains dust debris. All shelves need to be cleaned, sanitized and maintained.

55. Physical facilities installed, maintained & clean
- comments: observed basement of retail food establishment containing sleeping living quarters for 2 or more employees with telvisions, mattesses. Observed basement also contains dry storage room with food and food related items including to go containers. Living/sleeping quarters must be removed entirely or completely separated from retail food establishment with complete partitioning or solid self closing doors.

55. Physical facilities installed, maintained & clean
- comments: observed non working equipment including large 3 door reach in cooler not working, 1 door reach in cooler not working, cold hold unit, chest freezer and large oven. All non working equipment must be repaired or removed from premises. If equipment is to be stored it must be clean and sanitary and stored at least 6 inches off of floor and away from walls.
