38. Insects, rodents, & animals not present
- comments: 6-501. 111 observed 3 live roaches in the following areas: walls of the food prep area, shelving in the food prep area, and on the floor in front of the bar. Management instructed to clean and remove all activity from all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority violation #7-38-020(a).

38. Insects, rodents, & animals not present
- comments: 6-501. 112 observed many dead cockroaches inside of a glue board placed on the prep table in the food prep area. Also observed dead cockroaches on the floor of the food prep area. Management instructed to clean and remove all dead insects.

49. Non-food/food contact surfaces clean
- comments: 4-602. 13 observed soiled underside and splash areas of all kitchen countertops, hood line tables, equipment, and shelves, including between pieces of equipment throughout the kitchen and prep area. Instructed to detail clean all soiled areas. Clean the food debris and grease deposits inside of the ovens.
