- comments: observed fly strips hanging above hand washing sink, prep sink and ice machine in front smoothie prep area. Instructed manager to remove all fly strips from prep area.

- comments: observed over 100 mice droppings scattered on shelves under steam table, prep table, on bulk containers and canned goods in dry storage area, on floor under prep table and prep sink in smoothie prep area. Observed over 30 flying insects in front counter, smoothie prep, rear prep, storage and washroom areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued
