- comments: observed no written employee health policy on the premises. Instructed to provide and maintain signed employee health policy for each employee working on site. Priority foundation 7-38-010, citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the handwashing sink at front food prep area not accessible for use. Hand sink being used to store dirty containers and multi-use utensils and washcloths. Instructed manager, handwashing sink must be accessible for employees to wash hands at all times. Person-in-charge removed. Priority foundation violation 7-38-030 (c), citation issued.

- comments: observed black slimy substances on interior water components of the ice maker, dripping into ice. "ice is used for human consumption". Instructed management to turn off machine, remove ice and have interior of ice maker cleaned and sanitized and maintained. Priority foundation violation 7-38-005, citation issued.

- comments: observed no metal stem thermometer for taking food temperatures. Instructed to provide and maintain at all times. Priority foundation violation 7-38-005, citation issued.

- comments: observed bulk containers in rear without a label. Instructed to provide the common name on all bulk containers to properly identify item (salt, sugar, flour etc).

38. Insects, rodents, & animals not present
- comments: observed gap opening approx 1/4 inch at bottom right and left corner of rear exit door that leads to the outside garbage dumpster. Instructed to make door tight fitting all across the bottom to prevent pest entry and maintain.

Instructed to provide and maintain a licensed pest control logbook with all required documents (contract/service agreement, service report, chemicals used etc.). Also, provide at least 2 pest inspection reports each year and keep them in the pest control logbook for review. Priority foundation violation 7-38-020 (c), citation issued.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no chemical sanitizing test kit on the premises to measure the sanitizer concentration at the 3-compartment sink. Instructed to provide and maintain. Priority foundation violation 7-38-005, citation issued.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage dumpster with overflowing trash lids preventing proper lid closure and excessive litter around said dumpster area. Instructed to increase service pick-up or larger garbage dumpster must be able to close and remain tight fitting in order to prevent rodent harborage and maintain. Priority foundation violation 7-38-020(b), citation issued.

55. Physical facilities installed, maintained & clean
- comments: instructed to detail clean and organize the rear storage room/hallway/inside the water heater closet and remove unnecessary articles due to potential pest harborage and elevate mops, brooms cleaning equipments off the floor at least 6 inches to facilitate cleaning and maintain.
