- comments: observed a very thick brownish-pink and slimy substance inside the upper compartment of the ice machine dripping onto the ice used fro human consumption. Instructed to clean,sanitize and maintain. Priority foundation violation #7-38-005. Citation issued

- comments: observed frozen chicken and vegetables stored in plastic grocery bags in the coolers and freezers. Management instructed to use proper food grade storage bags and containers only.
49. Non-food/food contact surfaces clean
- comments: observed the interior of the one door freezer located in the basement to have an excessive amount of debris and ice buildup. Must remove excess ice, detail clean, and maintain.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacle in the womens toliet room. Instrcuted to provide and maintain.
