16. Food-contact surfaces: cleaned & sanitized
- comments: observed two ice machine interior drip plate heavily soiled with pink and black slime, located in basement and 2nd floor (outdoor storage area). Facility must deep clean and maintain. Priority foundation violation 7-38-005 citation issued.

38. Insects, rodents, & animals not present
- comments: observed over approximately 5 live roaches in the basement and 1 in the main cookline area. Observed dead roaches in the 2nd floor outside storage area. Facility must eliminate all dead/alive roaches in the facility. Priority foundaiton violation 7-38-020 (a) citation issued.

44. Utensils, equipment & linens: properly stored, dried, & handled
- comments: observed cooking pots with food contact surface facing upwards in direction of ceiling. Facility must invert pots to protect from contamination.

47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed cardboard used as lining for shelving in the walk-in cooler. Facility must replace liner with material that is smooth, easily cleanable, durable, and non-absorbent.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: equipment (large pots) used in the facility is too large to fit and fully immerse within the compartments of the three compartment sink. Facility must install a larger 3compartment sinkthat will allow all equipment to be immersed, use smaller equipment that can fit in the existing 3 compartment sink or use an alternative method of manual warewashing approved by the health department.

49. Non-food/food contact surfaces clean
- comments: observed caster wheels and exterior cookline equipment/in between units are soiled with food and grease debris. Facility must clean and maintain.

- comments: observed soiled floors (with food, grease and bottle caps) under the cookline and bar area facility must clean and maintain.

- comments: unnecessary items / nonfunctional equipment found in basement and 2nd floor (outdoor storage area). The premises shall be free of items unnecessary to the operation or maintenance of the establishment. This includes equipment that is nonfunctional or no longer used.
