1. Person in charge present, demonstrates knowledge, and performs duties
- comments: observed no person in charge with valid chicago food sanitation certificate.

2. City of chicago food service sanitation certificate
- comments: observed no certified food manager on duty while tcs foods are being prepared, handled and served such as (lunchmeat, sandwiches, soups etc) must be on site at all times. Priority foundation 7-38-012. Citation issued.

22. Proper cold holding temperatures
- comments: observed tcs foods held at improper internal temperatures in the walk-in cooler, such as 3lbs of chicken at 49. 1f and 3lbs of turkey at 49. 5f. The manager discarded and denatured a total of 6lbs at $50. All cold tcs food must maintain 41f or below. Priority violation. Citation issued 7-38-005.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: the walk-in cooler did not maintain the proper temperature. Observed to be 50. 1f. Instructed manager to repair and maintain cooler at 41f or below. The prep cooler is tagged held for inspection and must not be used until reinspected by the chicago department of health. Priority violation 7-38-005. Citation issued.

57. All food employees have food handler training
- comments: unable to provide food handler training certificates for all current food handlers. Instructed management that all employees who handle or prepare food must complete training. Must provide.
