- comments: 6-202. 15 observed a 1/4 inch gap at the bottom of the front entrance door. Instructed to make the front entrance door tight fitting and rodent proofed.

- comments: 5-501. 15 observed the outside garbage dumpster ground area with accumulated food debris (donuts, bagels). Instructed to remove food and maintain outside dumpster ground area at all times. Priority foundation. Citation issued 7-38-020(c).

55. Physical facilities installed, maintained & clean
- comments: 6-501. 114 observed bottles of bleach stored on the ground by the soda pop dispenser machine located in the rear food prep area. Instructed to elevate everything at least 6 inches from the ground and maintain.
