- comments: observed no certified manager from 2:45pm to 3:05pm while tcs foods such as chicken were prepared. At 3:00pm karla torres arrived on site. At 3:05pm certified manager karla torres certified manager certificate was delivered to another person on site through the back door on site. Instructed to maintain employee holding city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.
10. Adequate handwashing sinks properly supplied and accessible
- comments: observed handwashing sink inaccessible at the washroom area in operable due to them being disconnected and sign states no water. Instructed to repair or replace and maintain. Priority foundation violation 7-38-030(c)
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no toilet tissue available in the employees washroom at time of inspection. Instructed to provide at all times. Priority foundation 7-38-010. Citation issued
55. Physical facilities installed, maintained & clean
- comments: observed soiled mop heads not properly stored. Instructed manager to hang mop heads to prevent insect breeding.
57. All food employees have food handler training
- comments: person in charge unable to provide proof of training or food handlers certificates for any employee. Must provide and maintain.
58. Allergen training as required
- comments: observed no allergen training certificate on the premises. Management instructed that all certified food managers must have an allergen training certificate.
