22. Proper cold holding temperatures
- comments: observed the following tcs foods at improper cold holding temperatures during the time of inspection. They are as follows. . . Approximately 1. 5 lbs leafy spinach, 1 lb cooked eggs, 1 lb fresh mozzarella cheese, 2 lbs veggie patties. Noted temperature ranging between 50-53f. All stored inside cold hold unit in front prep. Instructed to hold all cold tcs foods at 41f or below at all times. Pic discarded voluntarily during this time. Priority violation. #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: during the inspection, approximately 25 small droppings were observed in the following areas on the premises: underneath the storage rack in the rear, scattered on the floor, inside a 2-door cabinet in the front prep area where napkins, prepackaged items, and crackers are stored, and inside the cabinets where the beverage dispenser is located. It was instructed to remove all droppings, clean and sanitize the mentioned areas, and eliminate the activity. This is a priority foundation violation. Citation has been issued.

51. Plumbing installed; proper backflow devices
- comments: observed leaky piping on front prep handsink. Instructed to repair and maintain in good condition.

55. Physical facilities installed, maintained & clean
- comments: observed floors in front and rear prep area floors with excessive food debris. Instructed to clean.

57. All food employees have food handler training
- comments: instructed to provide food handler training certificates for all food handlers. Must be maintained on-site for review.

58. Allergen training as required
- comments: all city of chicago's certified food managers must obtain food allergens and maintain on on-site for review.
