Report date: Mar 14, 20232. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on the premises while tcs foods (soup, deli meat sandwiches) were being prepared and served. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.
5. Procedures for responding to vomiting and diarrheal events
- comments: observed facility with use of the sample clean-up policy provided by the cdph website and no supplies. Must ensure the availability of both a clean-up policy and adequate supplies listed on posted policy (including disposable mop, absorbent material, disposable latex gloves, disposable mask, eye protection, and adequate sanitizer *must eliminate norovirus*) on site to properly clean up any vomit or diarrhea events that can potentially occur inside of the food establishment. Priority foundation violation #7-38-005. Citation issued.
33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed the left prep cooler inoperable. Instructed to repair the cooler so that it maintains 41f or below. Priority violation 7-38-005. Unit tagged and may not be used until reinspected by cdph and maintaining adequate temperature.
40. Personal cleanliness
- comments: observed a food handler without a hair restraint. Must provide and maintain.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed facility without a chemical test kit on site for the sanitizer (quat) being used for sanitizing at the three compartment sink. Informed the person in charge a test kit that accurately measures the concentration in mg/l of a sanitizing solution must be provided for the sanitizer being used. Priority foundation violation 7-38-005. Citation issued.
55. Physical facilities installed, maintained & clean
- comments: observed multiple broken wall base tiles broken near the mop sink and rear exit door. Must repair all broken tiles and maintain them.
56. Adequate ventilation & lighting; designated areas used
- comments: observed multiple lights burned out in the dish area and inside the walk-in freezer. Must replace all burned-out light bulbs and provide adequate lighting in said areas.
57. All food employees have food handler training
- comments: unable to locate food handler certificates. Instructed management that all food handling employees must show valid proof of training.
