- comments: observed no hand drying provisions at the following hand sinks 2nd floor boys toilet room, kitchen in annex building, instructed to provide and maintain. #7-38-030(c) no citation issued

- comments: observed during inspection the following items in dry storage in the annex building: open bottles of ketchup, mustard, and bbq sauce with mold-like substance; instructed to discard and maintain the integrity of products. #7-40-050(a) person in charge discarded products during inspection. No citation issued
38. Insects, rodents, & animals not present
- comments: observed approximately 35 small rodent droppings during inspection along the wall heating unit in the main lunchroom, also in the annex building kitchen behind the walk-in cooler, and dry storage shelving units. Instructed to remove droppings and clean said areas. Must eliminate activity on premise. #7-38-020(a) no citation issued

- comments: observed the following plumbing fixtures inoperable during the time of inspection: boys 1st floor toilet room 1 of 6 urinals, 1 of 4 hand sinks, 1 of 3 toilets cover with black garbage bags, girls toilet 2nd floor (1 of 7) toilet bowls, 2nd floor boys' toilet room 3 of 6 urinals covered with garbage bags, girls toilet room 1 of 7 covered and inoperable. Instructed to repair and maintain.

- comments: observed outside recpicale overflowing with garbage during time of inspection with lids unable to close tight. Instructed to remove and provide tight fitting lids to prevent pest harborage. #7-38-020 (b) no citation issued
