- comments: noted internal temperatures of the following time/temperature control for safety food items to be improper: 1. Inside the food display cooler: chicken 49. 4 f. 2. Inside one door refrigerator at the back prep area: pork 58. 8 f. 3. Back prep area by the cooking equipment: pork 62. 8 f. Manager immediately and voluntarily disposed of 219 lbs of products worth $420. 00 through denaturing process. Priority violation #7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: noted temperatures of glass door cooler at the back prep area to be improper at 78. 0 f and meat display refrigerator at 52. 8 f. Instructed not to use the units until equipment is maintaining proper temperature of 41f and below. A hold for the inspection tag has been placed on both units. Once repaired, fax or email to cdph. Priority violation #7-38-005. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed no current pest control log book on the premises. Instructed the person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year, and the reports must be maintained in the log book. Instructed to provide a current pest control log book, provider's current license, and pest inspection reports. Priority foundation 7-38-020(c). Citation issued.

38. Insects, rodents, & animals not present
- comments: observed pest activity with approx. 30 black flies were noted during the inspection. Fly activity was noted near the meat display refrigerator, wall near the 3-compartment sink, throughout the store, and in the rear prep area. Instructed to consult pest control as additional services are needed to minimize or eliminate activity before re-inspection. Priority foundation violation #7-38-020(a). Citation issued.

41. Wiping cloths: properly used & stored
- comments: noted dirty wiping cloths on prep tables and racks at rear prep areas not properly stored. Instructed properly to store all washcloths in a sanitizing solution, especially when not in use.

- comments: observed cutting boards with open seams and dark stains. Must refinish or replace damaged or stained cutting boards.

55. Physical facilities installed, maintained & clean
- comments: floors throughout the premise, especially underneath cooking equipment, with dirt, grease, and food debris and walls with food dirt and food spills. Instructed to thoroughly clean and sanitize floors and clean and maintain walls.

56. Adequate ventilation & lighting; designated areas used
- comments: observed missing protective shielding in employees' bathroom. Instructed to provide and maintain.
