- comments: observed calcium buildup on interior of ice machine in basement. Instructed manager to clean and maintain ice machine.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: observed no covered receptacles in women's toilet room. Instructed manager to provide covered receptacles in women's toilet room.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside dumpster receptacles overflowing with trash and unable to close. Instructed manager to ensure lids are closed and tight-fitting at all times. Priority foundation violation 7-38-020(b) citation issued.
