- comments: observed no certified manager on site while tcs items such as chicken were prepared. Instructed to maintain an employee holding a valid city of chicago foodservice manager certificate on site at all times. Priority foundation violation 7-38-012. Citation issued.

- comments: observed ice scoop stored directly inside ice machine. Instructed to store outside of ice machine to avoid contamination.

- comments: observed broken gasket on drawer cooler underneath flat top grill. Instructed to repair or replace and maintain in good condition.

- comments: observed build up of black slime on interior of air condiitoning unit near soda fountain. Instructed to detail clean and maintain in good condition.

- comments: observed some garbage and debris on ground in outdoor dumpster area. Instructed to clean and maintain in good condition.

- comments: observed build up of grease, grime, food debris, and dirt in the following areas: underneath soda fountain, inside fryer cabinet, inside microwave oven, and in rear corner near 2 door freezer. Instructed to detail clean and maintain in good condition.
