10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no paper towels at the front prep and dish area handwashing sinks. Instructed management all handwashing sinks must be equipped with a hand drying provision at all times. Priority foundation 7-38-030(c). Citation issued.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the dish area handwashing sink blocked with a metal container and tray inside of the sink basin. Also found the front prep area handwashing sink not maintained. Sink is inoperable; the drainage pipe below is missing and water sprays onto the floor when turning on the sink. Instructed management to keep all handwashing sinks accessible and free of dishes/items, and to repair the front prep sink. Priority foundation violation 7-38-030(c). Consolidated with above citation. Manager removed items from dish area handwashing sink.

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed the dish area handwashing sink with a hot water temperatures above 120f; cold water was turned off from underneath handwashing sink. Management instructed to maintain all handwashing sinks with a hot water temperature between 100-120f. Priority foundation 7-38-030(c). Consolidated with above citation. Management turned on cold water from underneath sink and the handwashing sink now provides hot water at 110f.

38. Insects, rodents, & animals not present
- comments: found 3 dead cockroaches behind the glass door cooler in the dish area. Must remove all dead insects from premises.

40. Personal cleanliness
- comments: observed two food handling employees preparing food without hair restraints. Management instructed that all food handling employees must wear a hair restraint while preparing food.

51. Plumbing installed; proper backflow devices
- comments: found a leaky ceiling pipe in the basement. Must repair.

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: found no covered waste receptacle inside the unisex washroom. Instructed to provide a covered waste receptacle (waste can with lid).

56. Adequate ventilation & lighting; designated areas used
- comments: noted heavy dirt and grease accumulation inside and on edges of the exhaust hood and filters above cooking equipment. Instructed to detail clean and maintain hood, filters, and pipes/conduits.

57. All food employees have food handler training
- comments: unable to locate and verify food handler certificates during inspection. Must provide and keep certificates on site.

60. Previous core violation corrected
- comments: previous core violation from 5/6/2021 report #2509306 not corrected: violation #58 (2-102. 13) observed certified food manager without the allergen training. Instructed above that all employees holding of chicago foodservice manager certificates must complete allergen training. Must correct previous core violations. Priority foundation 7-42-090. Citation issued.
