- comments: instructed to provide handwashing signage at handsink in bar area.

- comments: no splashguard in front bar hand sink and drainboard of 3 compartment sink. Must install a washable splashguard.

- comments: 4-501. 11---observed a torn door gasket inside of the 3 door prep cooler. Instructed to replace the torn door gaskets and maintain.

- comments: observed stained cutting boards in the kitchen prep areas. Instructed to resurface or replace the cutting boards.

- comments: instructed to detail clean food debris and grease spillage tops/sides/underneath the kitchen cooking equipment, refrigeration units, prep tables,storage shelves and food storage containers, interior and exterior of the freezers and prep coolers, deep fryer,interior microwaves, interiors of oven and deep fryers.

- comments: backflow prevention devices not located in basement both water hose connections underneath the dishwashing machine. Must install so backflow prevention devices may be located to be serviced and maintained.

- comments: observed water leaks underneath the kitchen dishwashing 3-compartment sink and behind the ice maker located in basement area. Instructed to repair plumbing leaks and maintain.

- comments: observed waste water backing up at the kitchen 3-compartment sink when water is running and draining at the far right compartment waste water backs-up into the far left and middle compartment of the kitchen 3-compartment sink and water slowly draining. Instructed to repair plumbing issue and maintain. Priority violation 7-38-030 (c), citation issued.

- comments: instructed to seal noticeable openings cracks/crfevices directly along and behind the basement chest freezer storage area and inside near the walk-in-freezer door.

- comments: instructed to seal noticeable openings on walls and ceiling areas throughout the basement dishwashing area,dry storage rooms,hot water tank room,unused office space areas and replace rotted ceiling wooden beams and repair broken ceiling areas in basement near the grease trap box also provide missing/broken ceiling tiles at rear near the mixer and in mens washroom.

Litter and shredded materials on floor in basement walk-in freezer. Must clean and maintain.

- comments: previous priority foundation violation #38 not corrected from report #2553287 on 03/23/2022: continue to observe evidence of pest activity on the premises; 10 rodent mice droppings on floors in walk-in-cooler and approximately 30 rodent (mice) droppings behind the chest freezers at rear storage area, approx: 60 or more rodent (mice) droppings throughout the kitchen behind-under-around the kitchen cooking/deep fryers/ovens/grills/stoves and refrigeration units and behind the pizza prep cooler, approx: 500 rodent(mice)droppings along the floors corners throughout the basement dishwashing/storage rooms/dry storage rooms/office space areas and on top of cluttered stored items in south-end of basement storage areas. Instructed to detail clean and remove all rodent (mice) droppings and sanitize all affected areas. Additional pest control service is needed to eliminate the pest activity. Priority violation 7-38-020(a),citation issued.
