- comments: observed tcs food items (cooked and prepared salsas) in reduced oxygen packaging in walk in cooler. Reduced oxygen packaging equipment being utilized on-site without valid haccp plan approved by cdph food protection division. Equipment tagged and held for inspection at this time. Instructed to remove tcs food from reduced oxygen packaging, discard, and denature, and cease use of reduced oxygen packaging equipment without haccp plan approved by cdph. Person-in-charge removed all tcs food, discarded, and denatured on-site. Approx. 10 lbs and $50 value discarded. 7-38-005 citation issued priority foundation

- comments: observed trailer outside in front of premises with cooking equipment and service window available to public to purchase food prepared on trailer. Trailer is not approved for any type of preparing/handling/selling of any items by city of chicago. Instd to remove trailer from premises. Also observed food handler using blender and preparing large batches of horchata and other beverages in dining room on tables and storing food items on floor. Instd to cease such operation in dining area and use approved preparation areas. Citation issued 7-38-005 priority foundation
