- comments: observed over 10 live roaches crawling on floor in front prep area under hand washing sink, on gasket or rear refrigrator, on floor under 3-compartment sink, prep tables, rolling carts, and shelving units in rear prep area. Instructed manager yo call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued

- comments: observed dead roaches inside and along door gaskets of reach-in cooler in rear storage area, on bottom of prep tables in prep area. Instructed manager to clean and maintain.

- comments: observed dead roaches on floor under coolers, cooking equipment, 3-compartment sinks, shelving units, and prep tables, along walls and in all corners in prep and storage areas. Instructed manager to clean and maintain.
