- comments: observed bulk containers not labeled in prep area. Instructed manager to label all bulk containers with common name of food.

38. Insects, rodents, & animals not present
- comments: observed over 60 mice droppings scattered on shelving units and on floor along shelving units and under 3-compartment sink in rear storage area. Observed numerous flying insects in prep, dish washing and storage areas. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a) citation issued.

- comments: observed 1/4" gap under front door. Instructed manager to seal all outer openings on door to prevent pest entry.

- comments: observed food items stored on floor behind front counter and in storage areas. Instructed manager to elevate all food six inches off floor.

- comments: observed food handler not wearing hair restraint. All food handlers must wear effective hair restraints.

- comments: observed soiled wiping cloths stored on prep table. Instructed manager to store wiping cloths in santizing solution between uses.

- comments: observed 2 oz containers used for dispensing condiments. Instructed manager to provide long handle dispensing utensils with handle above food to prevent contamination.

- comments: observed single service articles not inverted in prep area. Instructed manager to invert all single service articles to protect from contamination.

- comments: observed worn cutting boards with deep grooves. Instructed manger to replace cutting boards and maintain.

- comments: previous core violation not corrected from report #2619727 on 6-26-25:49-instructed to clean and maintain the following: fryers at bottom area, exterior surfaces of fryer and grill, interiors of coolers and freezers where needed. Meat cooler storing fresh meat and walk-in cooler have an odor from the fresh meat blood spills; clean and maintain. Clean fan covers inside of the beverage walk-in cooler to remove dust observed. Clean exterior surface of grease receptor at three comp sink to remove rust stains on top of receptor. 53-the washroom door is not self closing; instructed to provide a self closing device at washroom door and maintain. 57-instructed that food handlers must obtain the required food handler training. Priority foundation 7-42-090 citation issued
