2. City of chicago food service sanitation certificate
- comments: observed no city of chicago certified food manager on the premises while tcs foods (chicken, rice) were being prepared and served. Instructed management: a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

38. Insects, rodents, & animals not present
- comments: observed approximately 125 rodent droppings inside the premises in the following places: 40 mouse droppings inside the two ovens in the prep area that are currently not in use, 20 mouse droppings under the sink and behind the toilet in the employee washroom, and 65 rat droppings in the basement on the floor behind the storage shelving. Management instructed to clean and remove all rodent droppings from all affected areas and contact pest control to eliminate all pest activity from the premises. Priority foundation 7-38-020(a). Citation issued.

- comments: found a gap at the bottom and top of the front white door. Must seal gaps to prevent pest entry and maintain.

- comments: found heavy rust build up on the bottom shelves of the two tables in the dishwashing area. Must remove rust, so shelving is smooth and easily cleanable, and maintained.

- comments: observed torn gaskets on the 2-door true cooler in the prep area. Must repair or replace, and maintain.

- comments: found an accumulation of dirt, grease, and debris on the wok station, cooking equipment, inside both ovens, and the interior of the fryer. Must detail clean and maintained.

51. Plumbing installed; proper backflow devices
- comments: observed a pipe leaking from the ceiling in the basement. Must repair.

55. Physical facilities installed, maintained & clean
- comments: found dirt and debris on the floor underneath the cooking equipment in the rear prep area and on the floor throughout the basement. Observed standing water on the floor in the cook's line and dishwashing area. Must detail clean floors, remove any standing water, and maintain.

- comments: found poor lighting in the basement storage area. Must provide adequate lighting in said area and maintain.
