- comments: observed approx. 20 live small flying insects flying around behind the bar area and resting on the soda gun nozzles and bottles of liquor. Additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a). Citation issued.
48. Warewashing facilities: installed, maintained & used; test strips
- comments: observed no irreversible registering temperature indicator on the premises to measure the surface temperature of dishes being sanitized in the high temperature dish machine. Management instructed to provide. Priority foundation violation 7-38-005. Citation issued.
49. Non-food/food contact surfaces clean
- comments: clean the wall mounted potato slicer with food debris on it.
55. Physical facilities installed, maintained & clean
- comments: observed chipped paint on the ceiling of the food prep area in several areas. Management instructed to scrape and repaint the affected areas.

- comments: observed the certified food manager without an allergen training certificate. Management instructed that all certified food managers must show proof of allergen training.
