10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no handwashing signage at the exposed handsink behind the front bar. Instructed the person in charge to provide handwashing signage at all handwashing sinks used by food employees and maintain.

22. Proper cold holding temperatures
- comments: observed cold tcs foods in prep cooler at improper temperatures:ham 49. 6f, shredded cheese 50. 2f, sliced tomatoes 48. 6f, and beef 49. 8f. The person in charge volunatrily disposed of 40pounds tcs foods. Valueed at $200 dollars. Must maintain cold tcs foods at 41f or lower. Priority violation 7-38-005. Citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: observed prep cooler at main cookline not maintaining proper temperature 54. 8f. Instructed the person in charge to repair and maintain the prep cooler at 41f or below. The prep cooler is tagged held for inspection and must not be used until reinspected by the chicago department of health. Priority violation 7-38-005. Citation issued

38. Insects, rodents, & animals not present
- comments: observed pest activity with approx. 25 small flies noted during the inspection. Small fly activity was observed on the wall near the dish machine and the wall above coffe station. Instructed the person in charge to eliminate all fly activity, clean surfaces, maintain premises free of pest. Recommended receiving additional pest control service. Priority foundation violation 7-38-020(a). Citation issued.

39. Contamination prevented during food preparation, storage & display
- comments: observed food containers stored on the floor in the walk-in cooler in the basement. Instructed the person in charge to properly store and protect food items from contamination while stored by elevating at least six inches from the floor and maintain.

54. Garbage & refuse properly disposed; facilities maintained
- comments: observed outside garbage dumpster area overflowing, preventing lids from closing properly. Instructed the person in charge to ensure that trash is properly contained, not overflowing and that garbage lids are tight fitting. Priority foundation violation 7-38-020(b). Citation issued.

55. Physical facilities installed, maintained & clean
- comments: observed food derbis on the floor throughout prep areas. Instructed the person in charge to clean floors and maintain.

57. All food employees have food handler training
- comments: observed multiple employees with expired food handler certificates. Instructed the person in charge to renew expired food handler certificates to comply with idph food handler requirements.

58. Allergen training as required
- comments: observed one certified foodservice manager without allergen awareness training certificate. Instructed the person in charge to ensure that any individual that has a city of chicago foodservice managers certificate obtains allergen awareness training and certifcate.
