- comments: observed no city of chicago certified food manager on the premises while tcs foods (beef sandwiches, hot dogs) were being prepared and served. Also observed, no original city of chicago certificate was posted. Instructed management that a city of chicago certified food manager must be present at all times while tcs foods are being prepared or served. Priority foundation 7-38-012. Citation issued.

- comments: found cold tcs food at an improper cold holding temperature inside the 2-door true cooler. Observed a box of beef (pastrami) at 50f. Instructed always to maintain all cold-holding tcs foods at or below 41f. Priority violation 7-38-005. Citation issued. Food discarded.
38. Insects, rodents, & animals not present
- comments: observed no current pest control log book on premises. Instructed the person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year, and the reports must be maintained in the log book. Records must be kept for twelve (12) months and include a copy of the service tickets and the pest control company�s illinois department of health license number. Instructed to provideinstructed to provide a current pest control log book and pest inspection reports. Priority foundation violation. 7-38-020(c). Citation issued.
