- comments: observed bulk food storage containers in rear preparation area not properly labeled. (sugar, flour, salt etc. ) must provide the common name on all bulk containers to properly identif items.
38. Insects, rodents, & animals not present
- comments: observed evidence of roach activity on premises. Observed two live small roaches crawling on floor underneath storage shelving in the basement and one live roach on wall underneath dishwasher machine located on the first floor. Instructed to clean and sanitize areas. Additional pest control service is needed to eliminate roach activity problem. Priority foundation violation 7-38-020(a) citation issued.
40. Personal cleanliness
- comments: observed employees not wearing hair restraints while food is being prepared and served. Must wear hair restraints at all times food is prepared and served.
41. Wiping cloths: properly used & stored
- comments: observed wiping cloths stored underneath and on top of prep tables in kitchen food preparation area. Must properly store wiping cloths when not in use.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: observed plastic cups used as a scoop for dry foods. Must use durable utensils for scooping dry foods. Observed damaged doors gasket at the 3-door prep cooler. Instructed to repair damaged door gaskets and maintain.

- comments: observed accumulation of dirt and food debris on floor under around and behind all cooking equipment and dishwashing area. Must clean and maintain floor clean at all times. Must maintain floor dry on basement at all times.

- comments: observed several openings into ceiling around pipes on basement also observed peeling paint on walls on basement food storage area. Must seal and repaint walls surfaces must be smooth and easily cleanable surfaces.

- comments: observed mechanical ventilation in washrooms not working properly. Instructed to repair ventilation and maintain.
