- comments: no certified food manager on site while tcs foods were being prepared and served (milk, deli meat, cheese). Priority violation. Citation issued. 7-38-012.

- comments: observed large metal bin and garbage blocking hand sink in dish washing area. Hand washing sink must be accessible at all times for employees to wash hands. Priority foundation violation. 7-38-030(c). Citation issued. Employee removed items from hand sink during inspection.

- comments: observed no paper towels at hand washing sinks throughout establishment. Must provide paper towels at hand sink at all itmes to properly dry hands. Priority foundation violation. 7-38-030(c). See above for citation. Employee provided paper towels at hand sinks during inspection.

- comments: instructed to provide common name on bulk containers out of original packaging(candy, toppings).

- comments: observed pest control log book is incomplete. Last service reports were dated on 7-24-2023 and 10-28-2021. Pest control services must be provided at least twice a year by a licensed exterminator. Idph pest control license number and service reports must be maintained on site for a 12 month period. Priority foundation violation. 7-38-020(c). Citation issued.
