- comments: noted over 20 rat droppings on the floor along the wall base of the rear dry storage, bar, and dishwashing area. Instructed to detail clean and sanitize affected areas, eliminate pest activity. Instructed to consult with pest control company and have premises treated. Priority foundation #7-38-020(a). Citation issued.

- comments: noted rear exit door not rodent proof with gap at the bottom of door (about 1/4 inch). Instructed to seal gaps to prevent entry points for pests.

- comments: noted ice scoop stored inside ice machine on ice. Instructed to remove and provide an ice scoop holder.

- comments: instructed to detail all cooking equipment, inside the electrical unit of deep fryer, rails of refrigeration units with thick grease, old food particles, and dirt.

- comments: noted leaking faucet at bar handwashing sink. Instructed to repair and maintain.

- comments: instructed to detail clean and maintain floors along wall base especially throughout the kitchen prep and dishwashing area with thick grease and grim, standing water on the floor and dirt, must also clean and sanitize affected areas where rat droppings were found, floors inside the beer walk-in cooler. Recommended to upgrade housekeeping on the first floor.

- comments: noted holes on wall underneath handwashing sink in the food prep area and ceilings throughout basement. Instructed to seal all holes and openings to prevent entry points for pest.
