- comments: observed no hand washing signs or posters near hand washing sink in front bar area. Must provide and maintain.

- comments: person in charge unable to provide a pest control log book or service reports for review. Instructed to have a license pest control co. Service establishment and provide all required documentation for review including a current copy of pest control state of illinois license priority foundation violation 7-38-020(c). No citation issued.

- comments: observed no covered waste receptacle provided in the unisex washroom located in the 1st floor. Must provide and maintain.
