- comments: no vomit and diarrhea clean up kit on premises as required; must provide. Instructed the person in charge to provide a vomit and diarrhea clean up kit to use in the event of a vomit and diarrheal incident on premises. Priority foundation violation 7-38-005.

- comments: no exposed handsink located behind the bar for handwashing as required; must install with hot and cold running water, soap and handtowels. Priority foundation violation 7-38-030(c).

- comments: observed rodent activity on premises. Found rodent droppings on the floor in the corner area of the bar and behind the dj booth; instructed to clean areas and have a licensed pest control company service establishment. Priority foundation violation 7-38-020(a) citation issued.
