- comments: found approx 100 mouse droppings behind stove and cooking equipment. Also noted were mouse droppings in the rear garage area behind all coolers. Instructed manager to remove, clean, sanitize, and maintain said area. Recommended contacting pest control. Priority foundation violation. 7-38-020(a). Citation issued.

- comments: found 1/4 inch gap at rear exit door. Also noted 1/4 inch gap at garage door. Instructed manager to provide tight doors at all times to prevent pest entry.

- comments: observed no covered receptacle inside of unisex employee toilet room. Instructed to provide.

- comments: observed continued non- compliance from inspection report # 2522481 dated on 7/8/21: observed no allergen certificates on premises at this time, instructed to provide the following previous core violation remains in non-compliance pic does not have allergen certificate all city of chicago food service manager must have proof of allergen training. Instructed manager to provide. Priority foundation violation. 7-42-090. Citation issued.
