- comments: observed broken weather-strip at front entrance door and rear exit door with gap at bottom of door approx:1/4'. Instructed to make doors tight fitting all across the bottom to prevent pest entry.

- comments: observed approximately 20 rodent (rat) droppings in cluttered basement on the floor near the hot water tank location. Instructed to detail clean and remove all rodent (rat) droppings and sanitize all affected areas,remove cluttered conditions,additional pest control service is needed to eliminate the pest activity. Priority foundation violation 7-38-020(a),citation issued.

- comments: observed boxed paper supply items stored on the basement floor. Instructed to properly store and protect stock supply items from contamination while stored by elevating at least 6' off the floor on raised storage racks.

- comments: instructed to detail clean and organize basement storage rooms and remove unnecessary articles due to potential pest harborage and elevate mops,brooms cleaning equipments off the floor at least 6' inches to facilitate cleaning.

- comments: instructed to detail clean kitchen exhaust hood/filters to remove excessive grease build-up and maintain.

- comments: previous core violations not corrected from inspection report#2545052 dated:12/03/2021 #47--observed 2 cutting boards on hot line in kitchen covered in deep grooves and dark stains. Instructed to repair or replace and maintain smooth, cleanable cutting surfaces. #51--unable to locate grease trap. Instructed to install and attach to 3 compartment sink in dish area in accordance with all relevant laws, rules, and regulations. Must provide location of grease trap or install grease trap as required. #58--observed one certified manager without allergen training. Instructed all employees holding city of chicago foodservice manager certificate must complete allergen training and maintain records thereof on site at all times. Instructed to correct noted violations and maintain by 4/8/2022. Priority foundation violation #7-42-090,citation issued.
