- comments: observed no hot water at hand washing sinks in first floor unisex washroom(69. 6f), first floor girls washroom south (84. 1f) first floor boys washroom south (86. 2f), second floor unisex washroom (75. 2f),second floor boys washroom south (93. 2f), thrid floor boys washroom north (89. 4f), third floor girls washroom north (75. 2f). Instructed manager to provide at least 100f at all hand washing sinks. Priority 7-38-030(c)

- comments: observed over 60 mice droppings scattered on floor along walls behind grease trap, 3-compartment sink and cooking equipment, in corners in kitchen, 20 mice droppings scattered in corners of hallway leading to lunchroom, 20 mice droppings scattered in corners of lunchroom, 20 mice droppings scattered on shelves in dry storage area. Instructed manager to call an exterminator for service, clean and sanitize all areas. Priority foundation 7-38-020(a)

- comments: observed unused articles and equipment in unused lunchroom near kitchen. Instructed manager to remove all unnecessary items to prevent pest harborage.

Observed peeling paint on walls behind milk cooler, on ceiling and duct work in hallway, girls lower level washroom and staff second floor washroom.
