- comments: observed no certified manager on site when tcs food(oxtails 153. 8f, rice 149. 9f) prepared and served. A certified manager must be on site at all times tcs food prepared and served. Priority foundation 7-38-012 citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: observed no hot water 48f at hand washing sink in employee washroom. Still no hot water at hand washing sink from 3-11-21, 48 hours given on 3-11-21. Instructed manager to provide at least 100f water at hand washing sink. Priority 7-38-030(c) citation issued

39. Contamination prevented during food preparation, storage & display
- comments: observed food items stored on floor in rear storage area. Instructed manager to elevate all items six inches off floor. Observed cracked lid on bulk container. Instructed manager to replace.

49. Non-food/food contact surfaces clean
- comments: observed accumulated food debris, spills, grease and dust buildup on interior and exterior surfaces of coolers, freezers, holding units, shelving units, vanity cabinet in washroom. Instructed manager to clean and maintain.

55. Physical facilities installed, maintained & clean
- comments: observed food spills and debris on walls behind 3-compartment sink, rear hand washing sink, shelving units, prep tables, front counter area, etc. Observed accumulated debris on floor under all equipment, along walls and in all corners in front counter, prep, storage, and dish washing areas. Observed dust buildup on ceiling vents and ceiling tiles in prep area. Instructed manager to clean and maintain.

56. Adequate ventilation & lighting; designated areas used
- comments: observed damaged light shield in rear prep area. Instructed manager to replace and maintain.

60. Previous core violation corrected
- comments: previous core violation not corrected from report #2366406 on 3-26-20:58-instructed that managers with city of chicago food sanitation certificate must have allergen training. Must provide. Priority foundation 7-42-090
