- comments: noted dirty wiping cloths on prep tables at front and rear prep areas. Not properly stored. Instructed properly to store all washcloths in a sanitizing solution especially when not in use.

48. Warewashing facilities: installed, maintained & used; test strips
- comments: noted premise using a high temperature warewasher with gauge reading 185f; however noted no irreversible registering temperature indicator. Instructed to provide an irreversible registering temperature indicator, such as strips/stickers or thermometer. Priority foundation #7-38-005. Citation issued.

60. Previous core violation corrected
- comments: 8-404. 13(b:4) noted the following core violations 43, 47, 55 with the rules numbers below from inspection #2472510 dated 1/11/2021 not corrected:- -43)3-304. 12 noted serving plates on shelves at the kitchen prep area not properly stored. Instructed to invert all serving plates on shelves to prevent dust accumulation. -47) 4-501. 11 noted thick ice build up inside the beer walk-in cooler on the fan. Instructed to repair and maintain the condensation units. -55)6-201. 17 noted multiple holes and openings on ceilings, walls, and along with piping especially at the basement food prep/dry storage area. Instructed to seal all holes and openings to prevent entry points for pests. Priority violation #7-42-090. No citation issued.

On Sunday 6/23/24 I had a milk shake in the evening about 7pm. I woke up about 2am with pain in the intenstines, then minutes later had major diarrhea, and this continued for about 12 more hours. Fortunetly the diarrhea calmed down and I got normal again.  | Symptoms: Diarrhea
