1. Person in charge present, demonstrates knowledge, and performs duties
- comments: person in charge did not have a city of chicago sanitation certificate. Priority foundation violation.

2. City of chicago food service sanitation certificate
- comments: no city of chicago sanitation certificate posted on the premises or such a person on site when foods and mixed beverages were being prepared and served. Instructed a city of chicago certified food manager must on site during that time. Priority foundation violation 7-38-012 citation issued.

22. Proper cold holding temperatures
- comments: observed the following tcs foods held at improper temperatures. Tofu sandwiches in the front display cooler at 48. 2f and milk in the rear cooler at 45. 5. All discarded. Reviewed all tcs foods must be held at cold temperatures 41f or below. Priority violation 7-38-005 citation issued.

33. Proper cooling methods used; adequate equipment for temperature control
- comments: noted two coolers found at inadequate temperatures. Front display cooler ambient temperature at 52. 9f with tofu sandwiches at 48. 2f. Rear cooler at 57. 2f with milk at 45. 5f. Both coolers tagged held for inspection and must not use until repaired and maintains a temperature of 41f or below. Priority violation 7-38-005 citation issued.

37. Food properly labeled; original container
- comments: all pre-packaged prepared foods obtained from an outside food provider must be properly labeled with the business name and address of the facility where the food is prepared.

49. Non-food/food contact surfaces clean
- comments: all wooden shelving at the front prep and wooden countertop surrounding the front prep hand sink with water damage and in poor repair. Must replace.

- comments: no visible backflow at the utility service sink. Must provide.

- comments: damaged floors noted in the following areas: hallway to the washroom and office, open gaps in wooden floor. Damaged floor tiles under the rear prep hand washing sink and utility service sink. Instructed to repair to make smooth and cleanable. ----dirty floors throughout establishment with dirt debris and extremely sticky with beverage spillage. Must clean and maintain. ----walls throughout the rear cooler storage/utility sink area in poor repair with open holes, missing plaster and expanding spray foam as a sealant. Must repair all walls throughout rear to be smooth and cleanable.

55. Physical facilities installed, maintained & clean
- comments: unused ice machine in rear prep must be removed from the premises to prevent pest harboring.

56. Adequate ventilation & lighting; designated areas used
- comments: noted some dry foods being stored in the office under non-shielded lighting. Must provide light shield above all stored dry foods.

57. All food employees have food handler training
- comments: no proof of food handler training or certificates on site for all required employees. Must provide.

58. Allergen training as required
- comments: no proof of allergen training or certificate on site for the above certified food manager. Must provide for all city of chicago certified food managers.

60. Previous core violation corrected
- comments: previous core violations from inspection #2355602 on 12-16-19 not corrected. #36-4-204. 112(b)-observed no thermometer in mini fridge at front prep area. Instructed to provide. #51- 5-204. 12-no visible backflow prevention device at espresso machine in front prep area. Instructed to provide. #55- 6-201. 13-observed missing coving under mop sink in rear. Instructed to repair. ---- 6-201. 13- observed two cracks in-wall by rear prep table at dishwashing area. Instructed to seal, repair, and maintain. Priority foundation violation.
