- comments: : 6-202. 15---: outer opening not protected. Noted the mesh screening on the open kitchen window not attached. Must secure mesh screening to be tight fitting on all sides to prevent fly entry.

- comments: 4-901. 12---- must not hang wiping cloths for air drying on the splash guard of the hand washing sink.
47. Food & non-food contact surfaces cleanable, properly designed, constructed & used
- comments: : 4-101. 19--- must not use milk crates as a means of elevation in the dry food storage room.
51. Plumbing installed; proper backflow devices
- comments: 5-205. 15--- extremely low hot and cold water pressure in the women's staff washroom on the 1st floor. Must repair to have a greater water flow.
53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: 5-203. 12---- observed the kitchen staffs locker room toilet broken off the wall, lying on the floor of the toilet room. Open plumbing lines on the wall with dirt, deris, cracked and chipping wall plaster and plumbing piping. Instructed to repair replace the toilet. Priority foundation violation 7-38-030(c).

- comments: 5-501. 16--- must provide a waste receptacle at the cooks line hand washing sink.
55. Physical facilities installed, maintained & clean
- comments: : 6-201. 11--- chipping and peeling paint with noted holes in walls at the following areas: at the washbowl in the kitchen staff locker room, in both the men and women's staff washrooms on the first floor. Must repair all.
