- comments: observed openings at base of both outer entrance doors. Instructed to seal all outer openings to prevent pest entrance.

- comments: observed approximately 20 mouse droppings in facility. Observed mouse droppings in interior of cabinet in dining room in between fountain machines. Instructed to eliminate rodent activity, remove mouse droppings and clean affected areas. Recommended licensed pest control company service facility on or after 1/9/2026. Priority foundation violation 7-38-020(a) citation issued.

- comments: observed three small dead mice in interior of cabinet in dining room in between fountain machines. Instructed to remove dead mice and clean interior of cabinet.

- comments: observed stacks of buns stored on bread rack on floor in walk in freezer. Instructed to provide adequate shelving to elevate all food and nonfood items at least six inches off of floor for ease of cleanability.
