- comments: 6-501. 112 observed dead insects on the floor of the basement, the floor of the food prep area and inside of the cabinets at the front food prep area. Management instructed to clean and remove all dead insects from the premises.

- comments: 4-602. 13 clean the grease on the prep table next to the deep fryer. Defrost and maintain the interior of the 1 door refrigerator next to the boxes of fountain pop in the basement. Clean the steam table and interior of the oven. Clean the encrusted grease on the side of the grill and deep fryer.

- comments: 5-204. 12 must install a backflow prevention device on the mop sink in the basement that has an attached hose.

- comments: 5-501. 17 must provide a covered waste receptacle in the employee washroom.

- comments: 6-501. 11 repair the openings in the walls of the basement dry food storage room. Seal the openings around the 3 compartment sink at the walls and ceiling. Replace the particleboard above the 3 compartment sink and install a drywall ceiling or vented access panel above the 3 compartment sink.

- comments: 6-501. 12 clean and sanitize all walls in the food prep area especially around the dish washing area where live cockroach activity was observed.

- comments: 8-404. 13(b)(3) previous priority foundation violations not corrected from report #2472766 on 1/15/21: 38 - observed 6 live cockroaches on the floor and walls of the rear food prep area around the 3 compartment sink, behind the prep table, and in the basement around the hot water heater and 1 door freezers. Additional pest control service is needed to eliminate all pest activity from the premises. Priority 7-42-090. No citation issued.
