- comments: 3-304. 14 observed wet wiping cloths stored on top of the prep tables. Management instructed to store all wet wiping cloths in a container of sanitizer when not in use.

- comments: 4-101. 11 observed frozen food stored in a plastic grocery bag in the chest freezer in the basement. Management instructed to use only food grade storage bags or containers.

- comments: 8-404. 13(b)(3) previous priority foundation violations not corrected from report #2472766 on 1/15/21: 10 - observed no exposed hand washing sink installed in the front food prep area of the facility where coffee and drinks are prepared and served. Management instructed to install an exposed hand washing sink in this area. 38 - observed 12 live cockroaches on the floor and walls of the rear food prep area around the 3 compartment sink, behind the prep table, and in the basement around the hot water heater and 1 door freezers. Additional pest control service is needed to eliminate all pest activity from the premises. Priority 7-42-090. Citation issued.
