- comments: observed timed faucets not functioning properly in girls and boys washrooms. Water must run continuously for at least 15 seconds before reactivating faucet.

- comments: observed openings in screen door of kitchen. Instructed manager to repair/replace and maintain screen door to prevent pest entry.

- comments: observed milk crates used as shelving units in paper good storage area. Instructed manager to remove and provide adequate shelving for easy cleaning and pest control.

51. Plumbing installed; proper backflow devices
- comments: observed drainpipe leaking under 3-compartment sink and leaking hot water faucet in west second floor staff washroom. Instructed manager to repair and maintain.

52. Sewage & waste water properly disposed
- comments: observed waste water backing up inside compartments and overflowing through grease trap onto kitchen floor when water is released from compartments of 3-compartment sink. Instructed manager to call plumber and repair 3-compartment sink. Priority 7-38-030(c)

- comments: observed no covered waste receptacles in staff washrooms. Instructed manager to provide.

55. Physical facilities installed, maintained & clean
- comments: observed peeling paint and water damage on ceiling in pre-k washroom. Instructed manager to repair and maintain.
