1. Person in charge present, demonstrates knowledge, and performs duties
- comments: person in charge at the time of inspection does not have a city of chicago sanitation certificate. Priority foundation violation.

2. City of chicago food service sanitation certificate
- comments: no original and valid city of chicago food service sanitation certificate was posted nor provided on-site for the present manager on duty. Instructed to provide. A least one certified food manager must be on site when food is handled. Priority foundation violation:7-38-012 citation issued

10. Adequate handwashing sinks properly supplied and accessible
- comments: no soap provided at handwashing sink in prep and bar(first floor )areas. Soap dispenser at prep area unble to dispense soap(dead batteries). Upon my rquest employee provided soap sink in the prep area. Instructed to provided soap at all handwashing sink for proper handwashing. Priority foundation violation:7-38-030(c),citation issued.

- comments: must label dry food in plastic containers in prep area with common name

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: cover is needed at receptacle inside women's washroom(basement area)

- comments: not enought lighting inside wine the walk-in cooler in basement area. Must provide.

- comments: no food handler training certificate provided on site,instructed to provide

- comments: no allergen training certificates provided on site. Instructed to provide for all certified food service managers

60. Previous core violation corrected
- comments: previous core violations not corrected from 4-13-2020, report#2367102 #39)3-305. 11 waste pipe above the disposable utensils(cups, containers to go etc). Not protected. In basement prep/storage area, must be encased said pipes. Surface must be smooth, cleanable, and non-absorbent surface or relocate items #47) 4-202. 16 must not use milk crates as a means of elevation for stored items. #47) 4-201. 11 broken rubber gasket inside the small white reach-in cooler. Repair/replace said gasket. #55) 6-201. 11 noted rough and unfinished brick wall behind the bar area on the first floor and partial at the bar in the basement area. Instructed to refinish walls to be smooth and easily cleanable priority foundation violation:7-42-090, citation issued
