- comments: observed storing serving tray in handwashing sink in front prep area, while food handler frying chicken. Corrected on site. Serving tray removed from handwashing sink. Handwashing sinks must be accessible for hand washing only at all times. Priority foundation violation 7-38-030(c) citation issued.
16. Food-contact surfaces: cleaned & sanitized
- comments: observed black substances inside interior tubing water components of ice maker,dripping into ice 'ice is used for human consumption'. Instructed to turn off machine, remove ice and have interior of ice machine cleaned and sanitized and maintain. Priority foundation violation 7-38-005,citation issued.
38. Insects, rodents, & animals not present
- comments: observed small gap at base of front entrance door. Instructed to seal opening to prevent pest entry.
49. Non-food/food contact surfaces clean
- comments: observed nozzle of soda fountain machine with red buildupon surface. Instructed to detail clean soda fountain machine nozzle and maintain.
55. Physical facilities installed, maintained & clean
- comments: observed hole in upper wall in between ice machine and one door cooler. Instructed to smoothly seal hole for ease of cleanability. Observed cardboard used in place of a ceiling tile above shelving units holding and chemicals. Cardboard is not cleanable and is absorbent. Instructed to replace cardboard with ceiling tile.

- comments: observed light out in interior hood. Instructed to replace light bulb in interior of hood. Observed dimly lit walk in cooler. Instructed to increase light intensity in walk in cooler.
58. Allergen training as required
- comments: observed expired allergen training for certified city of chicago foodservice managers. Instructed to obtain allergen training for all city of chicago foodservice managers and maintain certification on premises.
