- comments: observed no person in on site with foodservice manager training

2. City of chicago food service sanitation certificate
- comments: observed no valid foodservice sanitation manager certificate on site while tcs foods are prepared and served (italian beef, sandwiches). Instructed person in charge that valid foodservice manager certificate as well as certified manager must be on site at all times. Priority foundation 7-38-012. Citation issued

38. Insects, rodents, & animals not present
- comments: observed facility without license pest control service and log book availible at time of inspection. Informed person in charged to obtain and maintain required pest control provider and maintain pest control service.

49. Non-food/food contact surfaces clean
- comments: must clean the interior of all reach in coolers and freezer to remove spills and or debris.

50. Hot & cold water available; adequate pressure
- comments: premises has no hot running water at the 3 comp sink nor hand sinks as required. Must provide and maintain. Priority violation #7-38-030(c)

53. Toilet facilities: properly constructed, supplied, & cleaned
- comments: must detail clean and maintain the fixtures in the washroom.

55. Physical facilities installed, maintained & clean
- comments: must detail clean the floors throught out premises to remove spills and debris.

- comments: no food handler certificate on site for food handler employee. Instructed person in charge that all food handler employees must receive training and certificate must be maintained on site
